Job summary

JAG Physical Therapy is seeking a licensed Physical Therapist in New Jersey to join a care-first outpatient orthopedic team. The role focuses on evaluating patients, creating and executing plans of care, and promoting positive recovery outcomes.

Qualifications

• Bachelor's or Master's degree required; Doctorate preferred.
• Licensed Physical Therapist in the state practicing in NJ.
• Completed state CEU requirements in NY/NJ.
• Experience/knowledge with an EMR software preferred.

Responsibilities

• Collect and record patient care data per guidelines reflecting reimbursement impact.
• Perform initial evaluation and daily notes; communicate with team.
• Prepare for evaluation per department policies and standards.
• Establish treatment plans based on evaluation results and goals.
• Collaborate with interdisciplinary team; reassess plan as needed.
• Provide education and training to patient and support system.
• Oversee PTA, ATC, and PT aides; participate in seminars.
• Promote JAG in the community through lectures and presentations.
