Job summary

JAG Physical Therapy seeks a licensed Physical Therapist in Nutley, NJ, to assess patients, devise treatment plans, and guide daily care. You will use your clinical skills to improve pain, function, and independence in an energized, patient-centered environment.

Qualifications

• Bachelor’s or Master’s degree required; Doctorate preferred.
• Licensed Physical Therapist in New Jersey practicing in.
• Completed state CEU requirement (NY, NJ) per state rules.
• Experience with EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision-making and teamwork.

Responsibilities

• Collect and record patient data per guidelines and payer requirements.
• Perform initial evaluation, daily notes, reassessments and discharge plans.
• Prepare for evaluations per department policies and standards of practice.
• Establish treatment plans and collaborate with the interdisciplinary team.
• Provide patient education and training for ADLs/IADLs and rehab activities.
• Oversee PTA/ATC/PT aides and participate in staff training and seminars.
