Job summary

JAG Physical Therapy seeks a dedicated Physical Therapist to join our outpatient team. You will assess patients, create and implement effective treatment plans, and help patients heal, reduce pain, and maximize functional independence daily.

Qualifications

• Bachelor's or Master's degree required; Doctorate preferred.
• Licensed Physical Therapist in the state of practice.
• Completed CEU requirements in NY or NJ.
• Experience with EMR software preferred.
• Excellent oral and written communication; strong clinical decision-making.

Responsibilities

• Collect and record patient care data following policies and guidelines.
• Perform initial evaluation, daily notes, progress reports, and discharge plans.
• Record daily billing and maintain productivity per department guidelines.
• Prepare for evaluations with patient history and diagnoses knowledge.
• Select and administer tests and measures; formulate diagnosis and plan of care.
• Collaborate with interdisciplinary team and patient support system; adjust plan as needed.
• Refer to other professionals when necessary and manage discharge planning.
• Educate patients and families; provide rehabilitative training and activities.
• Oversee PTA, ATC, and PT aides; participate in billing seminars and community outreach.
