Job summary

JAG Physical Therapy is seeking a licensed Physical Therapist to join our outpatient team. You will assess patients, create and adjust treatment plans, and collaborate with the interdisciplinary group to maximize function and pain relief.

You will educate patients, supervise staff, and utilize EMR tools to document progress while delivering evidence-based, patient-centered care in a supportive environment.

Qualifications

• Licensed Physical Therapist in NY/NJ with state permits.
• Must have completed CEU requirements for NY/NJ.
• Excellent written and verbal communication skills.

Responsibilities

• Collect and record patient care data per guidelines and billing rules.
• Establish and monitor treatment plans with interdisciplinary team.
• Provide patient education and supervise support staff.
• Deliver musculoskeletal interventions and exercise prescriptions.
