JAG Physical Therapy is seeking a licensed Physical Therapist to evaluate, treat, and progress patients within a comprehensive outpatient setting. You will develop individualized plans of care, document progress, and collaborate with a multidisciplinary team.

Qualifications

• Bachelor’s or Master’s Degree required; Doctorate degree preferred.
• Licensed Physical Therapist in NY and active license required.
• CEU requirements completed in NY/NJ.
• Experience with EMR software preferred.
• Excellent oral and written communication.
• Strong clinical decision-making skills.
• Positive, professional demeanor and teamwork.

Responsibilities

• Conduct initial evaluation and ongoing assessments to guide treatment plans.
• Develop and implement plans of care with measurable goals.
• Record daily notes and progress, and communicate with the care team.
• Coordinate with other providers for referrals and care continuity.
• Oversee PT aides and support staff in daily activities.
• Educate patients and families on exercises and home programs.
