Baywood Home Care seeks a Director of Growth and Community Relations to lead growth initiatives, expand the referral network, and strengthen community partnerships in Minnesota. This role reports to the President and drives strategic growth across healthcare referrals, partnerships, and outreach.

You will build relationships with hospitals, clinics, rehab facilities, social services, and county agencies, while guiding a data-driven approach to meet service capacity and quality standards.

Qualifications

• Experience in home care or healthcare is required.
• Proven track record in growth or business development.
• Leadership experience developing and carrying out organizational strategy.

Responsabilités

• Lead growth, business development, referral, and community engagement strategy.
• Develop referral relationships with hospitals, clinics, rehab facilities, senior communities, case managers, discharge planners, social workers, and county agencies.
• Develop strategic partnerships to increase Baywood’s visibility and reach.
• Manage a structured referral and business development pipeline from outreach to new client.
• Represent Baywood at community events, conferences, networking events, and presentations.
• Coordinate with operations, intake, scheduling, and clinical leadership to align growth with capacity.
• Set and track growth goals and KPIs, including referral volume and conversion rates.
• Report to the President on results, trends, and ROI of outreach and sponsorships.
