Job summary

Catholic Charities in Saint Cloud, MN, is seeking a Designated Coordinator for four Group Homes. You will lead a compassionate team, oversee care plans, and coordinate services to adults with developmental disabilities and mental illness.

The role requires supervisory experience in a residential setting, a valid driver’s license, and background checks. A strong commitment to mission-driven, quality care is essential.

Qualifications

• Three years of experience with DDMI populations; at least two in group residential setting; at least one year of supervisory experience.
• Valid driver’s license and ability to pass background checks.
• Experience coordinating care with families, medical professionals, and community partners.

Responsibilities

• Lead and support a team of Direct Support Professionals through coaching, team meetings, and performance feedback.
• Deliver hands-on care and develop care plans to ensure health, safety, and well-being.
• Coordinate care with families, medical professionals, and community partners.
• Manage operations including scheduling, budgeting, compliance, and home maintenance.
• Champion quality by ensuring documentation, medication administration, and client programming meet high standards.
• Provide direct services to clients.
