Job summary

MN Care Services in Saint Paul is hiring a Designated Coordinator to organize and deliver respectful, individualized support for people with diverse behavioral, developmental, and health needs. The role collaborates with staff to promote safety, skill development, and independence according to each person’s plan.

Responsibilities include supervising staff, conducting meetings, managing schedules, ensuring MAR reviews, documenting care activities, and supporting community participation.

Qualifications

  • One or more years of Designated Coordinator experience.
  • Ability to follow care plans and maintain timely documentation.
  • Pass MN DHS background study and required 245D training.

Responsibilities

  • Provide supervision and training for worksite staff.
  • Conduct monthly team meetings and maintain staff schedules.
  • Ensure medication orders and MAR reviews with nursing staff.
  • Document shift notes and data to support person-centered goals.
  • Transport individuals to destinations using company vehicle.
  • Demonstrate crisis intervention and de-escalation skills.
  • Maintain HIPAA privacy and confidentiality.

Job description

The Designated Coordinator - Saint Paul at MN Care Services will be responsible for organization and delivery of respectful, compassionate, and individualized support to people with diverse behavioral, developmental, physical health, and/or mental health support needs in home-based settings. The Designated Coordinator works alongside each staff member to promote safety for them and the person served by them; skill development, independence, and overall well-being, recognizing that behaviors are a form of communication. Services are delivered in alignment with each individual’s support plan and personal goals.

We have two (2) Designated Coordinator positions available for St. Paul!

Position Duties, including but not limited to:
Administrative
  • Provide supervision, training, and support for worksite staff
  • Conduct monthly team meetings to ensure consistent communication and process follow-through by staff
  • Ensure staff schedule is maintained in a timely manner; ideally, at least two weeks in advance for the month
  • Review staff timesheets for accuracy, and approve requests for time off in a prompt manner
  • Ensure medication is ordered and that MAR reviews are completed in partnership with Nursing staff in a timely manner
Person-Focused
  • Follow the person's care plan and ensure all applicable documentation related to them is completed timely and accurately, such as shift notes or medication administration reports
  • Collect accurate data to drive positive behaviors as part of the person's care plan
  • Provide problem-solving assistance and build confidence in person served by assisting with activities such as meal planning, grocery shopping, housekeeping duties, cooking, organizational skills, financial management, and self-care
  • Advocate for, support, and empower individuals served by providing general skill training where the skill will be used
  • Support development of decision-making skills and informed choices in all aspects of daily living, including selection of service providers, goals, location, home decor, leisure activities, roommates, and/or personal possessions
  • Support individuals’ desires to be involved in their community by transporting them to their intended destinations via company vehicle
  • Coach individuals served through escalations in behavior or mood, or through emergency situations
  • Maintain positive working relationships with teammates, Designated Manager, other MN Care Services staff, people served, as well as their responsible party and/or care team
  • Adhere to the Vulnerable Adult and Child Abuse and Neglect Policies and Regulations, the Health Insurance Portability and Accountability Act (HIPAA) privacy policies and maintain confidentiality of all information pertaining to people served, their families, and MN Care Services employees
  • Other cross-functional duties in or out of position scope, as necessary
Position Requirements
Education & Experience (must meet one):
  • Bachelor’s degree in a related field (e.g., psychology, social work, human services, or healthcare), OR
  • Associate’s degree + 2+ years of direct experience supporting individuals with significant behavioral or mental health needs, OR
  • High school diploma/GED + 3+ years of progressive experience in behavioral, residential, or mental health settings
Core Requirements
  • One or more years of Designated Coordinator experience
  • Age 21+
  • Valid driver’s license with a clean driving record
  • Ability to pass MN DHS background study (including fingerprinting)
  • Ability to complete and demonstrate competency in required 245D training
Highly Preferred
  • Experience in a 245D or HCBS setting
  • Hands-on experience implementing behavior support plans
  • Experience with structured data collection and documentation
  • Familiarity with person-centered and trauma-informed approaches
Professional Skills & Expectations
  • Demonstrated ability to consistently implement behavior support plans with fidelity under supervision
  • Strong understanding of behavioral principles (e.g., function of behavior, reinforcement strategies)
  • Proven de-escalation and crisis intervention skills, including appropriate use of trained physical interventions when necessary
  • High-level documentation skills: accurate, objective, and timely behavioral data and service notes
  • Ability to work independently while maintaining alignment with team direction and supervisory guidance
  • Strong interpersonal skills with the ability to build and maintain professional boundaries
  • Excellent organizational skills and ability to manage competing priorities in a fast-paced environment
  • Commitment to ethical practice, confidentiality, and person-centered care
  • Proficient with basic technology (EHR systems, email, Microsoft Office)
Physical, Behavioral, and Safety-Related Requirements
  • Tolerance for exposure to verbal aggression, loud noise, or unpredictable behavior from individuals
  • Frequent standing, walking, and moving throughout the day, including following individuals during transitions or community outings
  • Ability to use hands and fingers to operate a computer keyboard, mouse, telephone, and other standard office equipment
  • Ability to lift, carry, push, or pull up to 50 pounds occasionally (e.g. assisting with mobility, moving equipment, or intervening in crisis situations)
  • Frequent bending, stooping, reaching, kneeling, and crouching (e.g. assisting individuals with activities of daily living, working at their level)
  • Ability to react quickly and move safely during crisis situations or behavioral escalations
  • Manual dexterity for creating data, using paper or electronic documentation systems, and manipulating small objects or adaptive equipment
  • Ability to implement physical intervention or restraint procedures (if trained and required by the program), including safely holding positions or moving to protect oneself and others
  • Capability to maintain prolonged physical positions during crisis support (Standing, kneeling, or blocking)
  • Awareness and control of body positioning and space to prevent injury to oneself or others during challenging behaviors
  • Sufficient vision and hearing to observe clients and respond to cues, behaviors, or environmental risks
Pay and Benefits while working at MN Care Services
  • Position is exempt from overtime earnings; compensation range $50,000 to $65,000 per year. Actual annual base offered the the right candidate will be based on combination of knowledge, skills, abilities, and leadership experience
