Qualifications

• Required: Pharmacist - State of FL.
• Preferred: ASHP (Pharmacist).
• Preferred: PGY-1 residency; PGY-2 residency.

Clinical Pharmacist Responsibilities

The GI Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.

• Shift: 8:00AM - 4:30PM
