Job summary

BayCare seeks a TOC Clinical Pharmacist in Tampa, FL, for a full-time, hybrid role at St. Joseph's Hospital. The position focuses on discharge follow-up to reduce readmissions and on patient care to improve medication safety and disease state understanding. Licensure in Florida is required, with PharmD preferred. PGY-1/PGY-2 experience is welcomed.

Qualifications

• Florida Pharmacist licensure required.

Clinical Pharmacist Responsibilities :

The TOC Clinical Pharmacist shall support the mission of the hospital by aiming to reduce hospital readmissions through patient discharge follow-up. Perform clinical duties and engage in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourage provider follow-up and continuity of care following hospital discharge.

• Facility: St. Joseph's Hospital - Hybrid
• Location: Tampa, FL
• Status: Full time, exempt
• Shift: 8:00AM - 4:30PM
• Weekend Work: Occasional
• On Call: No
