AlignMed Partners seeks a full-time Advanced Practice Provider (NP or PA) for a split position between Hopemont and Cortland Acres in WV. The role offers base salary $109,000–$124,000 with incentive bonus, no on-call or weekend work, and a focus on geriatric care within long-term care facilities.

The incumbent will provide primary and urgent care, coordinate with Center Leadership and Medical Director, and participate in a comprehensive benefits program including CME, relocation, and wellness

Qualifications

• Graduate from an accredited Nursing program, RN licensed, NP with MSN and an Older Adult and/or Family NP track.
• PA must graduate from ARC-PA accredited program and pass PANCE for PAC designation.
• State licensure for PA/NP and relevant certifications (NCCPA for PA).
• Minimum two years’ experience with adult or geriatric populations preferred.

Responsibilities

• Provide primary and urgent care services to nursing center residents in collaboration with other providers.
• Collaborate with Center Leadership to meet center goals and align with AlignMed Partners mission.
• Work on-site with Medical Director/Attending Physician for patient care decisions.
