Qualifications

• The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner with a Master's Degree in Nursing and a concentration in Older Adult and/or Family NP.
• PA must be a graduate from ARC-PA accredited program and pass PANCE; PA licensure and NCCPA certification required.
• A minimum of two years experience working with adult or geriatric population is preferred.

Responsibilities

• The Advanced Practice Provider delivers primary and urgent care to nursing center residents, collaborating with other providers on-site.
• Works with Center Leadership and Medical Director to meet goals and priorities and supports mission of improving patient outcomes.
