AlignMed Partners is seeking a full-time Advanced Practice Provider for a split role between Majestic Care Hopemont Center in Terra Alta, WV and Cortland Acres in Thomas, WV. The provider will deliver primary and urgent care to nursing home residents, collaborating with leadership and physicians to ensure high-quality care.

A Nurse Practitioner with a geriatric focus (MSN) or a Physician Assistant with active licensure is required; DEA is needed where applicable.

Qualifications

• Graduate from an accredited nursing school; RN licensed; NP with MSN and family/older adult focus.
• PA must graduate from ARC-PA accredited program and pass PANCE; PA licensure and DEA.
• Minimum two years experience with adult/geriatric populations preferred.

Responsibilities

• Provide primary and urgent care to nursing center residents.
• Collaborate with Center Leadership and Medical Director on care plans.
• Support AlignMed Partners mission with high-quality care and compassion.
