Qualifications

• Licensed NP or PA in the state of employment.
• Experience in primary care: internal medicine, geriatrics, or family medicine.
• Current BLS/ACLS certification.
• Comfortable caring for Medicare-aged patients with comorbidities.
• Strong communication and interpersonal skills.

Responsibilities

• Provide outpatient primary care to seniors as part of a collaborative team.
• Deliver high-quality care while addressing medical and social needs.
• Engage with center teams to ensure comprehensive patient support.
• Travel within the market to multiple centers as needed.
