Qualifications

• Licensed to practice nursing or Physician Assistant in the state of employment.
• BLS/ACLS certification required.
• Experience in primary care: internal medicine, geriatrics, or family medicine.

Responsibilities

• Outpatient primary care responsibilities only.
• Collaborate with a collaborative care team including physicians, RN Clinical Managers, and center staff.
• Provide high-quality primary care to Medicare-aged patients with comorbid conditions.
