Qualifications
- Currently licensed to practice professional nursing or Physician Assistant in the state of employment, active and non-probationary
- Experienced in primary care: internal medicine, geriatrics, or family medicine
- Current BLS/ACLS certification
- Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions
Responsibilities
- Provide outpatient primary care responsibilities only
- Be part of a collaborative care team that may include Family Practice and/or Internal Medicine Physicians, RN Clinical Managers, and center staff
- Deliver high quality primary care, available to patients and addressing medical and social needs
- Participate in maintaining a patient-centered, compassionate care environment
