Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to Medicare populations at our neighborhood clinics in Brooklyn. You will join large care teams, focus on quality outcomes, and participate in a compensation model that rewards outcomes and preventive care.

New graduates are encouraged to apply. A Center Medical Director track may be available for highly qualified candidates, with opportunities to lead a center and collaborate with regional leadership.

Qualifikationen

• MD/DO degree and board certification in Internal/Family Medicine.
• Active, unrestricted state medical license is required.
• Active DEA license for prescribing medications.
• US work authorization is required.

Aufgaben

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.

Ausbildung

MD/DO (Medical Doctor or Doctor of Osteopathy)

Board Certification in Internal Medicine or Family Medicine
