Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based healthcare for local Medicare populations within a neighborhood primary care network. You will work within a center team to focus on quality of care rather than volume, with potential for a Center Medical Director role for qualified candidates.

Responsibilities include diagnosing patients, guiding primary care, managing medications, and maintaining EMR data.

Qualifications

• Must have Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree.
• Board Certification in Internal Medicine or Family Medicine or eligibility.
• Active, unrestricted State Medical License in the US.
• Active DEA license is required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data (e.g., Canopy, Greenway).
• Collaborate with regional and central leadership to meet health quality goals.
• Direct and manage the center care team with Practice Managers.
• Perform other duties as assigned.
