Job summary

Oak Street Health in New York seeks a Primary Care Physician to deliver equitable, value-based care to Medicare patients at our neighborhood centers. You’ll focus on quality outcomes over volume, working with large care teams to support preventive and chronic care, and improve patient experience.

New graduates are encouraged to apply; experienced clinicians may be considered for a Center Medical Director role.

Qualifications

• MD or DO degree and board-certified/eligible in Internal or Family Medicine.
• Active, unrestricted State Medical License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient EMR data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
