Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based healthcare to local Medicare populations within our neighborhood primary care centers. Providers focus on outcomes and preventive care, supported by a large care team to reduce hospital admissions and enhance patient experience.

New graduates are encouraged to apply. Experienced physicians may be considered for Center Medical Director roles, with a strong emphasis on team collaboration and care coordination.

Formación

• MD/DO graduate required.
• Internal Medicine or Family Medicine board certification or board eligible.
• Active, non-probationary, unrestricted State License.
• Active DEA license required.
• US work authorization required.

Responsabilidades

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
