Job summary

Oak Street Health in New York seeks a Primary Care Physician to deliver equitable, value-based care to local Medicare patients within our neighborhood centers. You will work with large care teams to focus on quality over volume, with a compensation model that rewards outcomes and improves patient experience.

Qualifications

• MD or DO degree required.
• US board certification in Internal or Family Medicine preferred (or board eligible).
• Active, unrestricted US state medical license and DEA license typical requirement.

Responsibilities

• Assess and diagnose patients at local Oak Street Health clinics.
• Oversee, direct, and administer primary care within value-based model.
• Prescribe and manage medications as appropriate and safe.
• Maintain patient records in EMR systems (Canopy/Greenway).
• Collaborate with regional leadership to meet quality goals and benchmarks.
• Lead and collaborate with center care team and Practice Manager.
