Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based healthcare to local Medicare populations across neighborhood centers. You will work within a large care team to focus on quality of care rather than volume of services.

New graduates are encouraged to apply. Senior physicians may be considered for Center Medical Director roles, with a strong emphasis on patient outcomes and collaboration with leadership.

Qualifications

• MD or DO graduate with board certification in Internal Medicine or Family Medicine (or eligibility).
• Active, non-probationary, unrestricted state license required.
• Active DEA license and US work authorization.
• Commitment to value-based care and patient outcomes.

Responsibilities

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned
