Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable value-based care to local Medicare populations across our neighborhood clinics. You will work in a center with a large care team, focusing on quality outcomes rather than volume, with competitive bonuses and a full-time schedule.

New graduates are encouraged to apply; senior roles such as Center Medical Director may be considered for qualified candidates.

Qualifications

• MD or DO graduate with Medical Doctor or Doctor of Osteopathy degree.
• Internal Medicine or Family Medicine board certification (or eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.
• Fellowship in Geriatrics (preferred).
• Experience with geriatrics and value-based care (preferred).
• Experience in outpatient primary care settings (preferred).

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain EMR data via canopy and greenway.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the care team.
• Other duties as assigned.
