Oak Street Health, part of CVS Health, seeks a Primary Care Physician to deliver equitable, value-based care to local Medicare populations at our Joliet centers. You will work with a care team to improve outcomes and reduce hospitalizations, while focusing on preventive medicine and patient experience.

Qualifications

MD or DO graduate required.
Active, non-probationary, unrestricted State License required.
Active DEA license required.
US work authorization required.
Board Certification in Internal Medicine or Family Medicine (or board eligible).
Fellowship in geriatrics and experience with geriatrics preferred.

Responsibilities

Assess and diagnose patients at our local clinics.
Oversee, direct, and administer primary care.
Prescribe and administer pharmaceutical treatments and medication.
Maintain patient EMR data.
Collaborate with leadership to meet health quality goals.
Direct and manage the center care team.
