Oak Street Health, a CVS Health company, is seeking a Primary Care Physician to deliver equitable, value-based care for local Medicare populations within our neighborhood clinics. You will work within large care teams to emphasize quality of care over volume, in a model designed to reward outcomes.

Qualifikationen

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate.
• Board Certification in Internal Medicine or Family Medicine (or board eligible).
• Active, non-probationary, unrestricted State License.
• Active DEA license.
• US work authorization.

Aufgaben

• Assess and diagnose patients at our local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned
