Job summary

Oak Street Health seeks Primary Care Physicians to deliver equitable value-based care to Medicare populations at neighborhood centers. Providers work with large care teams to focus on quality over volume and to reward outcomes within our value-based model.

Qualifications

• MD or DO graduate required.
• Internal Medicine or Family Medicine board certification (or eligible).
• Active, unrestricted State Medical License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics
• Oversee, direct, and administer primary care
• Prescribe and administer pharmaceutical treatments and medication
• Maintain Patient Electronic Medical Record data via canopy and greenway
• Collaborate with regional and central leadership to meet health quality goals
• Work with Practice Managers to direct and manage the center care team
• Other duties as assigned
