Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations. You will work within a neighborhood primary care center supported by a large care team to improve quality and outcomes.

New graduates are encouraged to apply. A Center Medical Director track may be available for highly qualified candidates. This is a full-time position with a comprehensive benefits package.

Qualifications

• Must be MD or DO with full medical licensure.
• Board certification in Internal Medicine or Family Medicine, or eligible.
• Active, unrestricted state license, non-probationary.
• Active DEA license.
• U.S. work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage medications.
• Maintain patient EMR data.
• Collaborate with regional leadership to meet quality goals.
• Direct and manage the center care team with Practice Managers.
• Other duties as assigned.
