Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care for local Medicare populations across our neighborhood centers. You will work within a large care team to prioritize quality over volume and participate in outcome-driven incentives and preventive medicine.

New graduates are welcome to apply. A Center Medical Director track may be considered for highly qualified candidates, with a focus on leadership and patient outcomes.

Qualifications

• MD or DO graduate.
• Board Certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State Medical License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data via canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned.
