Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable value-based care for local Medicare populations at our network of neighborhood primary care centers. You will work with a large care team to focus on quality care rather than volume, with a strong emphasis on outcomes and patient experience.

New graduates are encouraged to apply; a potential Center Medical Director path may be available.

Qualifications

• Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) required or equivalent.
• Board certification in Internal Medicine or Family Medicine (or eligible).
• Active, non-probationary, unrestricted State License required.
• Active DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data.
• Collaborate with leadership to meet health quality goals.
• direct and manage the center care team.
• Other duties as assigned.

Education

M.D. or D.O.
