Job summary

ChenMed LLC is seeking a Clinical Director to directly supervise and train PCPs at the assigned center, ensuring maximized core model execution across clinical quality, efficiency, outcomes, and clinician/patient satisfaction.

The role includes independent care for older adults with acute/chronic illnesses, coordination of care with a care team, and leadership in weekly SH/TCM sessions. Travel locally/regionally/nationally may be required for training and recruitment.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or a related specialty.
• Current, active MD licensure in the state of employment.
• Board eligibility in the terminal specialty; board certification preferred.
• Minimum of one year of clinical experience in geriatric, adult, or family practice settings.
• Completion of ChenMed training, including Masterful Conversations and meeting facilitation.
• Current DEA number for Schedule II–V controlled substances.
• Basic Life Support (BLS) certification within 90 days of employment.
• Fluency in English; additional language fluency for certain centers.
• Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).
• Willingness to travel locally, regionally, or nationally for training and recruitment.

Responsibilities

• Provide independent care for patients with acute and chronic illnesses in older adults, maintaining full accountability for patient outcomes.
• Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with complex needs.
• Leverage a care team including medical assistant, care facilitator, and care coordinator for office-visiting patients.
• Coordinate with transitional care teams, case managers, and resources for patients unable to visit the office.
• Lead weekly Super Huddle and Transforming Care Meeting sessions.
• Support Center Manager when the clinical leader is unavailable; monitor daily census and champion outcomes.
• Assume clinical leadership duties, including recruiting patients and interviewing providers.
• Mentor and coach PCPs to achieve outstanding clinical outcomes; sample huddles and rounds.
• Partner with Center Operations Director to improve center financial performance and increase membership.
• Perform additional duties as assigned by management.
