Job summary

ChenMed is seeking a physician leader to independently provide care for older adults with acute and chronic conditions. You will lead the care team, coordinate across hospital, SNF, LTC, and homebound settings, and collaborate with hospitalists and market leadership to improve patient outcomes. Expect mentorship of PCPs and active center governance participation.

MD or DO with board eligibility and active state licensure is required; DEA and BLS within 90 days; travel may be required as needed.

Qualifications

• MD/DO with relevant board specialty and active license
• 1 year geriatric, adult or family practice clinical experience preferred
• Board eligibility required
• DEA license
• BLS certification within 90 days
• Willingness to travel as needed

Responsibilities

• Provide care for older adults with acute/chronic illness
• Lead care team including MA, care facilitator, and care coordinator
• Coordinate care in hospital, SNF, LTC, or homebound settings
• Engage with hospitalists and market leadership to improve outcomes
• Mentor PCPs and participate in center governance and performance

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar

Current, active MD licensure in state of employment

Board certification in Internal Medicine, Family Medicine, Geriatrics or similar (preferred); Board Eligibility required

DEA number for schedule II-V substances

BLS certification (AHA or Red Cross) within 90 days
