Job summary

Qualifications

• MD/DO in internal medicine, family medicine, geriatrics or similar; active state licensure.
• Board certification preferred; board eligibility required.
• DEA number for schedule II-V substances is required.
• BLS certification within the first 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Lead care team: medical assistant, care facilitator, and care coordinator.
• Coordinate with hospitalists, transitional care, case managers, and other resources.
• Lead weekly SH and Transforming Care Meeting (TCM) and support center leadership.
