Job summary

ChenMed is seeking a Clinical Director to lead and train primary care providers in a busy center. You will supervise clinical teams, enhance quality and outcomes, and allocate time between direct patient care and leadership duties to foster a high-performing culture.

The role requires MD/DO credentials, active state licensure, and board certification or eligibility in relevant areas. You will travel locally and participate in strategic clinical initiatives to improve patient care and center

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Current, active MD licensure in the State of employment.
• Board certification or eligibility in Internal Medicine, Family Medicine or Geriatrics.
• DEA number for schedule II–V substances.
• BLS certification within 90 days of employment.

Responsibilities

• Independently provides care for patients with acute and chronic illnesses encountered in older adults.
• Take full accountability for patient care and outcomes; seek consultation as needed.
• Engage with the hospitalist when a patient is in the hospital, regardless of employer.
• Oversee assessment, diagnosis, treatment, education, care coordination and documentation.
• Lead care team and coordinate care for patients able to come to the office.
• Engage with transitions for patients unable to visit the office (hospital, SNF, LTC, homebound).
• Lead weekly SH and TCM meetings; support center/market clinical leadership as available.
• Assist in recruiting patients and additional providers for the center.
• Mentor PCPs to deliver outstanding clinical outcomes and participate in daily huddles.
• Collaborate with Center Operations on financial performance and center membership growth.
