Job summary

ChenMed LLC is seeking a Clinical Director to supervise and train PCPs at a ChenMed center, driving high-quality care for older adults. The role combines leadership with direct patient care and ongoing clinical improvement.

You will mentor PCPs, coordinate across care teams, and help improve center performance while ensuring strong patient outcomes and satisfaction. Some travel may be required for training and recruitment.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
• Current, active MD licensure in the state of employment.
• Board certification in the relevant specialty preferred; board eligibility is required.
• DEA number for schedule II–V controlled substances.
• Basic Life Support (BLS) certification from AHA or ARC required within first 90 days.

Responsibilities

• Provide independent care for patients with acute and chronic illnesses among older adults.
• Take full accountability for patient care and outcomes, seeking specialist consultation as needed.
• Engage with the hospitalist when any patient is in the hospital.
• Conduct assessment, diagnosis, treatment, management, education, health promotion, care coordination and documentation for complex chronic needs.
• Lead the care team (care promoter, care facilitator, care coordinator) for patients able to come to the office.
• For patients unable to come to the office, coordinate with transitional care teams and other resources.
• Lead weekly Super Huddle and Transforming Care Meeting, and support center clinical leadership as needed.
