ChenMed LLC is seeking an Associate Clinical Director to supervise and train PCPs at a center, enhancing clinical quality, efficiency, and patient satisfaction. You will allocate time for direct patient care while directing reports and leading care teams in a physician-led model.

Qualified candidates will have an MD/DO in internal medicine, family medicine, or geriatrics, active licensure, and 1+ year geriatric clinical experience.

Formación

• MD/DO in Internal Medicine, Family Medicine, or Geriatrics.
• Current, active MD licensure in the state of employment.
• Minimum of 1-year clinical experience in geriatric, adult or family practice (preferred).
• Board certification in the terminal specialty (preferred); board-eligible is required.
• Completion of ChenMed training, including Masterful Conversations and meeting facilitation.
• Basic Life Support (BLS) certification.
• Current DEA number for schedule II-V controlled substances.
• Proficiency with Microsoft Office Suite and related applications.
• Willingness to travel locally as needed, nationally for initial training, and occasionally regionally and nationally for recruiting or training.
• Fluency in English; fluency in other languages may be required for some centers.
• Ability to exercise independent judgment.

Responsabilidades

• Supervise and train primary care providers (PCPs) in assigned center.
• Allocate time to direct patient care as a PCP and other duties based on direct reports.
• Maximize core model execution, improving clinical quality, efficiency, outcomes and satisfaction.
• Provide care for patients with acute and chronic illnesses in the older adult population.
• Seek consultation from specialists when appropriate and maintain detailed patient care.
• Engage hospitalists, transitional care teams, case managers, and other resources.
• Lead care team (medical assistant, facilitator, coordinator) for office-based patients; engage transitional care team for others.
• Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly and support center leadership as needed.
• Monitor daily center census and manage center operations, covering for other providers.
• Recruit patients and interview additional providers as required.
• Be available for walk-in visits and prompt communication to build patient trust.
• Deliver patient-centered, evidence-based care and continually improve clinical and operational performance.
• Promote physician wellness and self-care to sustain high quality care.
