AltaMed Health Services Corp. in Covina, CA invites applications for a Social Worker, MSW to join the PACE program. The role provides psychosocial evaluation, care planning, coordination, and discharge planning as part of an interdisciplinary team.

The successful candidate will hold an MSW, have at least two years of relevant experience with geriatric populations, and may need bilingual skills. A valid BLS certification is required.

Qualifications

• Master's Degree in Social Work (MSW) is required.
• Minimum of 2 years of experience in case management, social advocacy, and/or mental health, with geriatric population.
• Bilingual Spanish/English, Chinese/English, or another second language is strongly preferred.
• A valid BLS certification or higher is required.

Responsabilités

• Provide psychosocial evaluation, assessment, care planning, coordination of care, and disenrollment processes for the PACE program.
• Offer interventions including individual and group modalities, family and collateral contacts, education, counseling, resource mobilization, and discharge planning.
• Collaborate with the Interdisciplinary Team to optimize health status and quality of life for PACE participants.
