Job summary

Genesis is seeking a Social Services Specialist at our nursing center to assess and address the social, emotional, and mental health needs of patients and residents, coordinating services to enhance overall well-being and quality of life.

You will work with families and the care team to develop and implement an interdisciplinary plan of care, arrange discharge planning, and connect with community resources to support residents’ needs in a compassionate, compliant environment.

Qualifications

• Bachelor's degree in social work or human services required.
• Certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.

Education

Bachelor's degree in social work or human services field
