Genesis is seeking a Social Services Specialist at our nursing center in New Mexico. You will work with patients and families to identify social, emotional, and mental health needs and coordinate services to support well-being and quality of life.

You will collaborate with an interdisciplinary team, help create care plans, conduct meetings, and assist with discharge planning, referrals to community agencies, and ongoing support.

Formación

• Bachelor's degree in social work or human services field required.
• Certifications/licensures as required by State of employment to practice in long-term care.

Responsabilidades

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
