Qualifications

• Bachelor's degree in social work or human services required.
• State-specific certifications/licensures to practice in long-term care may be required.
• Ability to collaborate with interdisciplinary team and families.

Responsibilities

• Collaborate with interdisciplinary team to promote patient rights and wellbeing.
