Responsibilities include collaborating with the interdisciplinary team, developing a written care plan, conducting meetings, and coordinating discharge planning and community referrals.

Qualifications

• Bachelor's degree required.
• Certifications/licensure as required by state for long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote patient/resident well-being and rights.
• Develop an interdisciplinary plan of care to address social service needs for each patient/resident.
• Hold meetings with patients/families to provide support and information to improve quality of life.
• Coordinate discharge planning, education, and referrals to appropriate agencies.
• Establish relationships with community-based agencies/services to support residents.
