Job summary

Therapy Management Corporation (TMC) is seeking a Speech Language Pathologist to deliver personalized therapy to adult and geriatric patients, focusing on communication, swallowing, cognition, and independence. Qualifications

• Licensed Speech Language Pathologist (SLP) or license eligible in practice state.
• Graduate of an accredited SLP program.
• Interest or experience in geriatric rehabilitation or senior living therapy.
• Passion for patient-centered care and quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized speech therapy services to improve communication, cognition, swallowing, and quality of life.
• Develop and implement patient-centered treatment plans for speech, language, cognition and swallowing rehabilitation goals.
• Deliver evidence-based therapy interventions to support functional independence in adult and geriatric patients.
• Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care.
• Maintain accurate clinical documentation, monitor progress and adjust treatment plans as needed.
