Job summary

Asian Health Services is seeking a Geriatric Services Manager to lead PACE social work services, guiding psychosocial assessments, care planning, counseling, and referrals across home, center, hospital, and community settings. The role requires oversight of staff, regulatory documentation, and collaboration with the interdisciplinary team to ensure culturally competent, high-quality care for seniors.

Qualifications

• Master’s degree in Social Work required; accredited program.
• Minimum two (2) years of health-related experience.
• Leadership and/or management experience.
• Minimum one (1) year of experience working with a frail or elderly population; training if needed for PACE.
• Bilingual ability (Cantonese and/or Mandarin) and culturally/linguistically appropriate services.
• Social work competencies, leadership, judgment, and guiding staff in a team-based care environment.
• Knowledge of community resources, care coordination, documentation standards, and participant/caregiver support.
• Ability to provide respectful, participant-centered service to older adults.

Responsibilities

• Lead IDT assessments, care planning, and reassessments for PACE participants.
• Oversee delivery of social services, care management, counseling, referrals, and advocacy.
• Supervise Geriatric Services Specialists; manage staffing, scheduling, and performance.
• Develop and implement care plans addressing medical, social, emotional, and spiritual needs.
• Coordinate 24-hour care delivery, discharge planning, and home/hospital visits.
• Maintain PACE EHR documentation per federal regulations and policies.
• Act as liaison among participants, families, and service providers to ensure continuity of care.
• Educate participants, caregivers, and staff on aging, dementia, grief, and PACE delivery model.
