Qualifications

• Master's degree from CSWE-accredited social work program.
• Minimum one year experience with frail and elderly; 3+ years preferred.
• Experience conducting psychosocial assessments, care planning and case management.

Responsibilities

• Conduct social work assessments to determine psychosocial needs and actively participate in IDT meetings to develop participant care plans.
• Deliver and document social work interventions including arranging resources, care transitions, individual and group counseling, and case management.
• Completes initial assessments, re-assessments and care plan updates while evaluating ongoing social service and caregiver support needs.
• Participate in behavioral management meetings with other mental health professionals and coordinate with the care team for smooth transitions between settings.
• Maintain HIPAA-compliant documentation and provide referral support to community resources; develop relationships with community agencies.
