Job summary

PACE Southeast Michigan seeks a Social Worker Lead (LMSW) to assess needs, coordinate services, and deliver social work care within the Interdisciplinary Team. You will manage daily clinical activities, supervise staff, and ensure compliant documentation and care planning.

You will conduct bio-psychosocial assessments, provide individual and group counseling (CBT/DBT), and contribute to program development, discharge planning, and quality improvement efforts at the center.

Qualifications

• MSW from an accredited school and LMSW licensure in Michigan.
• Minimum three years of clinical social work experience.
• Experience supervising/leading a team and coordinating care.
• Knowledge of CBT/DBT and bio-psychosocial assessment.
• Proficient with EHR documentation and team-based care.

Responsibilities

• Conduct comprehensive bio-psychosocial assessments and risk screens.
• Provide individual and group counseling using CBT/MI techniques.
• Supervise social workers and interns; oversee care plans and LOCDs.
• Document thoroughly; participate in IDT meetings and quality projects.
• Coordinate discharge planning and interdisciplinary collaboration.
