Job summary

Neighborhood Healthcare’s PACE program seeks a Social Work Manager to lead social work services, supervise staff, and ensure high-quality, person-centered care while maintaining CMS and DHCS compliance.

This on-site role partners with leadership and clinical teams to optimize workflows, address barriers to access, and support program growth, with emphasis on trauma-informed care and culturally responsive service delivery.

Qualifications

• Current BLS certification required upon hire and maintained.
• AHA CPR and First Aid certification required for all PACE employees.
• Three years of social work with elderly and frail adults; four years preferred.
• Two years supervisory experience; elder population preferred.
• CA Driver’s License and auto insurance required.
• ACSW preferred (or ability to obtain).
• Completion of NHC LEAP Program preferred.

Responsibilities

• Provides leadership, supervision, and support to Social Work staff, fostering accountability and collaboration.
• Coaches and mentors staff to support professional development and clinical excellence.
• Monitors staffing patterns, caseload distribution, and service coverage for efficiency.
• Oversees IDT processes and coordination of care across teams.
• Ensures adherence to CMS and DHCS requirements and quality standards.
• Leads process improvements to enhance participant outcomes and workflows.
• Maintains documentation quality and audit readiness.
