Job summary

Providence Health and Services is seeking a social worker to provide direct services to participants in its geriatrics-focused program. You will conduct assessments, contribute to integrated care plans, and connect participants with community resources to ensure continuity of care across settings.

Join a multidisciplinary team across centers, clinics, and home visits. A Master’s in Social Work and Washington state social work licensure are required, with experience in geriatric populations

Qualifications

• Master's Degree in Social Work required.
• Washington Clinical Independent Social Worker Associate License or Temporary License required upon hire.
• 2 years social work with geriatric age groups from diverse ethnic cultures.
• 1 year work experience with geriatric population.

Responsibilities

• Provide social work services to participants and conduct direct service activities.
• Perform social work assessments and contribute to the development of individualized integrated care plans.
• Access community and financial resources and ensure continuity of care across settings.
• Coordinate care across care settings including home visits as needed.
• Collaborate with interdisciplinary team in center-based, clinic, and home environments.
