AltaMed Health Services in Long Beach is seeking a dedicated Social Worker to provide psychosocial evaluation and care planning within the PACE program. You will be integral in assessing participant needs and advocating for effective resource mobilization.

The ideal candidate will possess a Master’s Degree in Social Work and at least 2 years of experience with the geriatric population. Bilingual skills in Spanish/English or Chinese/English are preferred. Benefits include competitive insurance and career development opportunities.

Qualifications

• Minimum of 2 years of experience in case management, social advocacy, or mental health.
• Field work/internship may substitute 1 year of experience.
• Valid BLS certification or higher required.

Responsibilities

• Provide psychosocial perspective to interdisciplinary teams.
• Conduct assessments, care planning, and coordination of care.
• Perform case management and mobilize resources.
