Job summary

Qualifications

• Active CA Physical Therapist license in good standing.
• BLS/CPR certification required.
• Minimum 1 year PT experience with geriatric population.
• Strong written and verbal communication.

Responsibilities

• Screen and evaluate rehabilitation needs per APTA guidelines.
• Collaborate with nursing home staff on rehab plans.
• Document POC and coordinate with other therapies.
• Provide PT treatments and home exercise instructions.
• Educate patients and caregivers on DME usage and safety.
• Maintain accurate patient charts per policy.
