Job summary

Habitat Health in California seeks a Physical Therapist to provide evaluation and treatment across acute, restorative, and maintenance levels for PACE participants. You'll join the Interdisciplinary Team, develop individualized plans, conduct home visits, and educate participants and caregivers on exercise and DME.

Qualifications

• Bachelor’sDegree in physical therapy and active CA license.
• Completion of an APTA-accredited program or CMS-approved equivalent.
• Minimum one year experience with frail or elderly individuals.
• Current CPR/BLS certification and ability to work in a fast-paced environment.
• Strong learning mindset, data-driven decision making, and integrity.

Responsibilities

• Provide PT evaluation and treatment across acute, restorative and maintenance care for PACE participants.
• Develop individualized treatment plans and conduct home visits as needed.
• Educate participants and caregivers on exercise programs and DME use.
• Contribute to IDT care planning, documentation and regulatory compliance.
• Support DME inventory management and participate in quality audits.

Education

Bachelor's Degree in Physical Therapy

APTA-accredited physical therapy program or CMS-approved equivalent

Active Physical Therapist license issued by California

CPR/BLS certification
