Job summary
Neighborhood Healthcare is seeking a licensed Physical Therapist to provide evaluations and treatment plans for geriatric participants within our PACE program. You will integrate therapy plans into the plan of care under provider orders and collaborate with the interdisciplinary team.

Qualifications
- Valid CA Physical Therapist license in good standing required.
